- EduCare Summary of Benefits - $100 policy year deductible - 2009-10 - This document contains a summary of the standard EduCare benefits for schools that choose the $100 policy year deductible option.
- EduCare Summary of Benefits - $250 policy year deductible - 2009-10 - This document contains a summary of the standard EduCare benefits for schools that choose the $250 policy year deductible option.
- EduCare Claim Form - 2009-10 - Form used to seek reimbursement for covered medical, dental or vision expenses that have been paid to providers. Please submit to Worldwide Expatriate Administrators - or handle this entirely on-line via your personalized EduCare International Portfolio.
Aetna Global Benefits
- AGB Enrollment/Change Form - Form used to enroll members during the policy period (includes addition or termination of dependents). To avoid medical underwriting, enrollments must be received within 30 days of the date the individual is eligible for cover.
- AGB Claim Form - Form used by members to seek reimbursement for covered medical, dental or vision care expenses they have paid to providers for services. Please complete the form, include your email address and submit to AGB for processing.
Evacuation, Repatriation & Salary Continuation Applications
- AIG K&R Application - Includes Evac/Repat/Salary Continuation - Use this form for Group Coverage of Kidnap & Ransom, as well as Evacuation/Repatriation/Salary Continuation due to Political Instability.
- ACE K&R Application - includes Evac/Repat/Salary Continuation - Use this form as requested by your ISSFIN representative. Group Coverage for Kidnap & Ransom, including Evacuation, Repatriation and Salary Continuation due to Political Instability.
Errors & Ommissions (Professional Liability) Applications
- School Leaders E&O Application - This form is used to capture key information about your school that ISSFIN needs to secure quotations for School Leaders' Errors and Ommissions Insurance Cover.
Travel Insurance Application Forms
- Travel Medical Application Form - This form is used to capture the expected travel exposure information about your school that ISSFIN will need to secure travel medical insurance quotations on your behalf.
